I have lost the document for my State Savings product (including Prize Bonds). What can I do?

Prize Bonds

If you have lost your Prize Bond certificate and wish to retrieve details of the Prize Bond numbers please write to: State Savings, Prize Bonds, Fexco Centre, Killorglin, FREEPOST, Co. Kerry, V93 WN9T. Please list any variations of names and addresses including any previous addresses that may be relevant.

We will issue a response letter containing your Prize Bond numbers to your registered address. As it is optional to return Prize Bond certificates for repayments/transfers, it will not be necessary to issue a replacement certificate. Simply provide details of the Prize Bond on the repayment/transfer form.

If you are a registered State Savings Online customer, you may sign in and obtain your Prize Bond numbers from your holdings view.

Fixed Term products

If you have lost your documentation, you will be required to complete a Lost Document application form. The form must include the required customer details, the relevant account number, it must be signed, witnessed and then returned along with the proof documentation listed on the Lost Document application form. The form must be witnessed by either a Post Office official, a member of An Garda Síochána, or a practising Solicitor or Commissioner of Oaths.

At the end of your State Savings product’s term, we will write out to you with a Maturity Option Form which gives the option of re-investing or repaying your Savings product. At this point you can indicate on the back of the Maturity Option Form if the ownership document is lost. If it is lost you must sign the relevant section and get an appropriate witness to fill out the witness panel on the back of this form along with the requested proof documentation. In the case of more than one person being named on the account, each named person must sign and provide proofs. The form needs to be witnessed by either a Post Office official, a member of An Garda Síochána, or a practising Solicitor or Commissioner of Oaths.
